1012 - Outside Agency Death Notifications
1012.1 PURPOSE AND SCOPE
As a professional courtesy, the Coroner Division will occasionally assist outside agencies by performing in-person death notifications on their behalf. This policy provides instruction for the Deputy Coroner receiving the death notification request.
1012.2 REQUIRED INFORMATION
Outside agencies may submit their request for a death notification by a telephone call directly to the Coroner Investigations Unit. Upon receiving the request, the Deputy Coroner shall obtain the following information:
- Requesting Party's name, agency, contact number, and case number
- Decedent's Name, date of birth, and date of death
- The method of identification
- Brief circumstances of death
- Name, relationship to decedent, address, and phone for the person to be notified
1012.3 CASE NUMBER PREFIX
After obtaining all required information, the Deputy Coroner shall memorialize the agency assist with an "Assist" case number, designated with the prefix "A."
- Example: A-001-24 All "Assist" cases are documented via a log maintained at the Coroner Division.
1012.4 PERFORMING THE DEATH NOTIFICATION
The outside agency death notification shall be performed at the discretion of the Deputy Coroner, taking into consideration the circumstances of the death. Any death notification performed on the behalf of an outside agency shall be performed in accordance with Policy Section 1103.4 of this manual.
1012.5 INFORMING OUTSIDE AGENCY OF OUTCOME
The Deputy Coroner performing the outside agency death notification shall be responsible for communicating results of that notification attempt back to the requesting party. If next-of-kin notification was successful, the Deputy Coroner shall provide the requesting party with a name, address, and contact number for the next-of-kin.
1012.6 DEATH NOTIFICATION REPORT
The Deputy Coroner shall document the results of the outside agency death notification in RIMS via a call for service.
Assist Outside Agency
